Personal Wellness/Home Health Aide

Full Time position available, must be able to work rotating weekends and holidays

Job Summary:

Provide assigned residents with routine daily care and services in accordance with the resident’s assessment and care plan, and as directed by supervision. Ensure compliance and policies/standards are upheld.

Some Responsibilities and Duties as an Personal Wellness/Home Health Aide:

  • Observe and report all changes in the resident’s condition to the Care Coordinator as soon as practical.
  • Thoroughly read and comply with daily printed assignments sheets as determined by the Personal Wellness Coordinator. These assignments may include:
    • Assist with transferring residents, as necessary. Assist with lifting, turning, moving, positioning and transferring into and out of beds, chairs, bathtubs, wheelchairs, lifts, according to care plan and/or standards.
    • Assist residents with daily dental / mouth, eye/ear care, toileting / bathing, dressing/undressing and other hygiene or daily activity functions according to care plan.
    • Assist residents in preparing for medical tests, social activities.
    • Weigh and measure residents as instructed. Measure and record temperatures, pulse and respirations, as instructed.
    • Assist with care of dying resident and provide post-mortem care, as instructed.
    • Provide transportation for residents to various community locations. Comply with state and federal DOT procedures.
    • Collect and appropriately label soiled laundry.
    • Distribute clean personal laundry as needed.
  • Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm, safe environment throughout the shift.
  • Assist with housekeeping, meal serving and ensure room comfort.
  • Maintain an effective, friendly working relationship with health professionals, physicians, consultants, and family members.

Qualifications and Skills:

  • Prefer experience in an acute care, long-term care, assisted living community or home care for elderly.
  • CNA and PCT Certification preferred.
  • Ability to deal tactfully and communicate with team members, residents, family members, visitors, government agencies/personnel, and the general public with patience and a cheerful disposition.
  • Ability to work extended or flexible hours, as necessary.
  • Ability to read, write, speak and understand the English language.
  • Willingness to handle difficult situations involving residents.
  • You must be able to work independently as well as a part of a highly productive team.
