Admissions Nurse-RN/LPN

Department: Center for Health and Rehab
Location: East Lansing, MI

Full Time Position Available.  Typical schedule will be 4 days a week,10 hour shifts and on-call rotation for weekend coverage.

Position Summary:

Conduct assessment of new resident upon admission.  Perform resident treatment and preventative medical treatment, as appropriate.  Provide documentation.   Ensure compliance and standards are upheld. 

Essential Duties and Responsibilities:

Assessment/Orientation

  • Meet with new resident and family members upon arrival to community for general admission orientation ensuring a friendly welcome to the community.
  • Complete admission duties and paperwork on the floor/hall (neighborhood) where the new resident is assigned to minimize confusion.
  • Complete physical assessment, uptake forms, medication reconciliation from the discharge summary, initial care plans, and other forms relevant to the admission process. 
  • Clarify all ambiguous medication/treatment orders at the time of admission. 
  • Complete the MARs at the time of admission and have all transcriptions. Have them reviewed and double checked by another nurse or nurse manager prior to placing them in the med books. 
  • Have a clear understanding of how ICD-10 coding impacts reimbursement and drives the necessary changes to workflow and related documentation.
  • Be versed/trained on ICD-10 diagnosis codes as the primary reason for the patient’s stay, and a determinant of reimbursement.
  • Be proficient in ICD-10 coding to generate an updated, accurate picture of the resident’s current health status at the point of admission to the community.
  • Regular visits to the hospitals, as needed to meet with potential residents or discharge planners.
  • Communicate daily with the Admissions Manager/Coordinator to prepare for the current and following day’s admissions.
  • Report any complaints or grievances made by residents and maintain documentation.  
  • Assist in monitoring the inventory of medications, supplies and equipment to ensure that an adequate supply is on hand and to ensure appropriate dispensation.

Clinical / Compliance

  • Work on floor as needed when admission duties are completed to provide appropriate assessment, diagnostic tests, treatment and other charge nurse responsibilities.  Document thoroughly and report findings appropriately.
  • Provide surveillance of medical regimens for chronically ill persons, adjust regiments within established protocols, and recognize when to refer to physician.
  • Maintain updated resident assessments/care plans.  Evaluate and make recommendations to Director and others, as needed, regarding resident change in condition and/or placement.  /serve on the care plan team and work with the team in developing a comprehensive assessment and care plan for residents. 
  • Make written and oral reports/recommendations to attending physician, Medical Director and/or Administrator/Director concerning the status and care of the assigned resident.

Marketing / Admissions

  • Develop relationships with external community members to promote goodwill for Burcham Hills.
  • Make presentations to small or large groups as identified.
  • Visit referral sources and develop personal connections with hospital visits doctor office visits, events, and other activities.
  • Cross-train for admissions process support and perform as necessary

Position Specifications:

Education and Experience:

  • Possess and maintain a valid, current and unencumbered nursing license for the State of Michigan.
  • Prefer RN.
  •  Clinical experience as a nurse, in an acute care, long term care, assisted living community or other related health care setting.
  • Valid driver’s license in good standing.

Knowledge and Abilities:

  • Ability to make independent decisions within scope of practice when circumstances warrant such action.
  •  Ability to deal tactfully and communicate with team members, residents, family members, visitors, government agencies/personnel, and the general public with patience and a cheerful disposition.
  •  Ability to work in a multi-faceted role with fast-paced demands.
  •  Ability to work with little or no supervision and be self-directed to ensure priorities are established and follow through is conducted.
  •  Ability to work extended or flexible hours, as necessary, and work on-call shifts according to Department protocol.
