Wellness Navigator/Social Worker

Department: Resident Center
Location: East Lansing, MI

Burcham Hills is an upscale senior living retirement community located in the backyard of Michigan State University in East Lansing, MI. We currently have a vacancy for a Wellness Navigator to work a Full Time schedule. In this position a Wellness Navigator/Social Worker will be responsible for building relationships, solving problems and locating resources for residents transitioning throughout the life plan community. The Navigator will evaluate residents’ needs and assist them in accessing the available resources needed to ensure a seamless transition between appropriate levels of care offered at the community, in addition to but not limited to the following duties:

  • Coordinate the welcoming of new residents and families in the Resident Center and ensure a smooth transition to the community.
  • Interview residents and families to obtain social history. Evaluate information to assist in determining appropriate services, activities and interventions.
  • Leads interaction and communication with the resident, family members, physicians and staff members when a change in the resident’s condition necessitates additional services or a physical move within the continuum of care.
  • Collaborates with Center for Health and Rehab or external skilled nursing staff to coordinate residents’ short-term and long-term health center stays, including admissions, discharges, and supportive services.

Qualifications: Social Work or Clinical Assessment background is a plus! Three to five years of experience working in long-term care, post-acute care or other health care setting is preferable along with progressive duties in organizational or management experience. Prefer supervisory experience in health care or a long-term care facility with skilled and support staff. Ability to work extended or flexible hours, as necessary. Ability to read, write, speak and comprehend written and oral instructions in English. You must be able to work independently as well as a part of a highly productive team.
